Services Connect is for vulnerable and disadvantaged Victorians with one or more support needs, who require a level of assistance to address problems they are experiencing.
It has a focus on helping people to achieve positive change by building their strengths and capabilities.
Support needs might include housing, employment, learning and development, cultural and social wellbeing, health, safety and issues related to risk behaviours.
The model offers a more integrated way of supporting people to meet their personal goals. It is designed to connect people with the right type and level of support, by considering the whole of person or family’s needs across a range of outcome areas.
* Services Connect is being trialled in numerous locations across Victoria. Hume Moreland Services Connect commenced in 1 November 2014 and will run over two years until 31 October 2016.
